چکیده

Background Systemic sclerosis (SSc) is characterized by widespread tissue fibrosis including the myocardium. Diffuse myocardial fibrosis can be detected non-invasively by extracellular volume (ECV) imaging based on preand postcontrast T1 measurements using cardiovascular magnetic resonance (CMR). We hypothesized that multiparametric CMR, including T1 mapping, can detect subclinical myocardial involvement and provide a comprehensive cardiac assessment in patients with SSc.

Early markers of heart damage in systemic sclerosis: role of cardiac magnetic resonance with late gadolinium enhancement

Background Cardiovascular complications are one of the main causes of death in patients affected by systemic sclerosis (SSc). Heart involvement is characterized by myocardial fibrosis, arrhythmias, conduction system defects and pericardial inflammation. Also patients with subclinical cardiac involvement have a poor prognosis: thus early markers of heart damage are necessary to identify patients...

Subclinical myocardial inflammation and diffuse fibrosis are common in systemic sclerosis – a clinical study using myocardial T1-mapping and extracellular volume quantification

BACKGROUND Systemic sclerosis (SSc) is characterised by multi-organ tissue fibrosis including the myocardium. Diffuse myocardial fibrosis can be detected non-invasively by T1 and extracellular volume (ECV) quantification, while focal myocardial inflammation and fibrosis may be detected by T2-weighted and late gadolinium enhancement (LGE), respectively, using cardiovascular magnetic resonance (C...

Impaired myocardial perfusion is associated with extracellular volume expansion, disease activity and impaired strain and strain rate in systemic sclerosis: a cardiovascular magnetic resonance study

Background Systemic sclerosis (SSc) is characterised by vascular dysfunction and multi-organ fibrosis, with the heart commonly involved. Cardiovascular disease (CVD) in SSc may be direct or indirect, but often remains subclinical. SSc patients with apparent cardiovascular clinical features are at greater risk of deterioration and premature cardiovascular death, often from complications of myoca...

Global and regional extracellular volume and myocardial circumferential strain correlate significantly in a heterogeneous patient population

Background Myocardial extracellular volume (ECV) quantification by cardiovascular magnetic resonance (CMR) T1 mapping technique has shown great promises in detecting diffuse fibrosis and interstitial diseases of the myocardium. In hypertensive and left ventricle hypertrophy (LVH) patients, increased ECV has been found to be associated with impaired myocardial strain, which is an early indicator...

Diffuse myocardial fibrosis is subclinical and is associated with impaired myocardial deformation characteristics in systemic lupus erythematosus: a cardiovascular magnetic resonance study

Background Systemic lupus erythematosus (SLE) is a systemic autoimmune disorder that commonly affects the heart, resulting in a 7 to 9 times greater incidence of cardiovascular disease (CVD) in SLE patients compared to healthy controls. Female patients with SLE between 35 and 44 years old have an incidence of myocardial infarction over 50 times greater than that observed in the Framingham cohor...
